heat transfer problems

Problem (1) 100% cotton white shirt tranfers - edges are not embedding - running machine @ 375 and 10-12 seconds/cooling for 5 seconds/ peeling slowly.

Problem (2) 100% cotton black shirt transfer using same method above - ink looked like it just got too hot(molten like) as I was peeling off paper, but trnsfer did not embed it bubbled.

Problem (3) I have ruined 5 shirts in the process, can you buy test cloth somewhere.
